Medical Records

Patients or their legal representative with proper documentation may inspect, obtain copies of, or have copies of their medical records.

Please print and complete the appropriate form:

• Medical records release form in English
• Medical records release form in Spanish
• Authorization for Disclosure of the Result of the HIV Antibody Blood Test
• Authorization for the release of Alcohol or Drug Abuse Test
• Authorization for Inspection of health information

You can request your Medical Records by the following three ways:

   Attn: Medical Records
    
3. Request in person
   Office hours: 9am to 12noon & 2pm to 4pm
   -Please allow at least 5 to 10 days to complete your request
   -Same day pickup may not be available

Other important information about inspecting or requesting a copy of your Medical Records:

• You may only inspect records within the medical records department and there may be a retrieval charge
• There may be a reproduction charge for creating a copy of your records
• If the reproduction charges are greater than $100, then pre-payment will be required before your records can be mailed

Copy services copying hours are by appointment only.
